All forms must be ledgibly completed before student-athletes may participate in Intercollegiate Athletics at Erskine College.
Student-Athletes and Parents,
As the head athletic trainer, I consider it my privilege to provide athletic training services to you and your sons or daughters. To allow me and my staff to provide the best athletic training services, we would ask for you to complete and sign the following forms and fax them back to the Erskine College Athletic Department by August 1st, 2007. The fax number is located at the top of this letter. All forms require Adobe Acrobat Reader in order to be downloaded.
Please complete the following forms as legibly as possible:
6. Physical Examination (do not need to send if you had a physical done during the spring at Erskine)
